to the address above.
Also tell us which size card you want,
business card size or neck wallet size, inside the self
addressed stamped envelope. Note, Medical Alert
Wallets sends 2 Medical
Information Cards and a flyer of our wallets, unless
requested different, up to 4 cards max.
